First Visit

Your initial appointment will consist of an evaluation and explanation of your diagnosis and treatment options. Occasionally, surgery can be performed the same day as the evaluation. However, a complex medical history or treatment plan may require a consultation appointment to provide you with the most appropriate course of treatment. If you were told by our office prior to your visit that IV Sedation might be a possibility for treatment please read the information on our pre and post surgical instructions.

Please assist us by providing the following information at the time of your evaluation:

  • Your surgical referral slip and any x-rays if your dentist gave them to you.
  • A list of medications you are presently taking.
  • If you have medical or dental insurance, bring the necessary identification cards. This will save time and allow us to help you process any claims. For your protection, we also require you to bring a photo I.D.

IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the evaluation and procedure visits.

A preoperative evaluation and physical examination is mandatory for patients undergoing IV anesthesia for surgery.

Please alert us if you have a medical condition that may be of concern prior to surgery (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).

X-Rays

If your dentist or physician has taken x-rays, please request that they forward them to us. If additional films are necessary, they can be taken at our facility. Many of our referring doctors have access to a secure link to our practice to send the referral and x-ray to us directly.
